Service Line Material Submission Information
If you have any questions, call our office at 701.869.2690 or visit us at 217 Main Ave, Gilby ND between the hours of 8 AM to 4 PM Monday – Thursday.
The EPA is REQUIRING all water systems to keep a record of the material used for all service lines. To do this Agassiz Water is requesting that you fill this survey out which includes answering the following questions about your service line and providing us with a picture of your meter along with the pipes coming into and going out of the meter.
Options are listed to make a submission of information:
1. Fill out paper form and mail it to the office or drop it in our lock box on
our door.
2. Contact the office during business hours (8 AM – 4 PM) by phone with
your information.
3. Walk into office during business hours (8 AM – 4 PM, Monday –
Thursday) with your information and picture and I will help you.
4. Email this information to office@awud.net.
